Abstract: 

Background: Prostate cancer is the third leading cause of death and is the most common cause of cancer in elderly men. Regarding to the low accuracy of screening methods such as prostate-specific antigen (PSA), Digital Rectal Examination (DRE) and trans rectal ultrasound (TRUS) in detection and localization of tumor, Magnetic Resonance Imaging (MRI) and Diffusion Weighted Imaging (DWI) attracted many attentions in the past years. DWI reveals micro-molecular Diffusion, which is the Brownian motion of the spins in biologic tissues. This technique can delineate pathologic lesions with high tissue contrast against generally suppressed background signal. In this paper, the value of DWI in detection of prostate cancer is studied.Methods: In this cross-sectional study, the studied population are suspicious patients to prostate cancer based on high Prostatic Specific Antigen level or abnormal Digital Rectal Examination who refered for prostate biopsy to radiology department of Hazrate- Rasoul Hospital during the year 2011. The results of DWI are compared to biopsy results for all patients.Results: Eighty five patients are selected. The DWI sensitivity in detecting of prostate cancer is 100%, specificity 97.1% and positive and negative predictive values are 89.5% and 100%, respectively. The results showed that if DWI reports the prostate cancer as negative, the result was highly reliable and if it reports as positive, although the report was not 100% reliable, but it still had high reliability, more than 90%.Conclusion: DWI had high accuracy in detecting prostate cancer for patients with cancer. Also the accuracy of this method for patients without prostate cancer was acceptable compared to the other common methods.

Abstract: 

Background: Multiple sclerosis (MS) is a chronic disease that begins most commonly in young adults and is characterized pathologically by multiple areas of central nervous system (CNS) white matter inflammation, demyelination, and glial scarring. The most valuable aid for diagnosis is magnetic resonance imaging (MRI). New type of MRI has been developed on the basis of molecular Diffusion which capable of detecting acute and active lesions. Early diagnosis and treatment is possible to stop or slowdown progression of disease. The aim of this study is to compare the findings of conventional and Diffusion- weighted (DW) MRI in detection cerebral lesions of MS.Methods: Thirty patients with clinically definite MS and 30 healthy volunteers were studied in a descriptive-prospective survey over a twelve-month period of time.Conventional and DW MRI were used in both groups. Total number, morphology, location and the mean size of the intra-cerebral plaques of MS were compared between group. The sensitivity and the specificity of both imaging methods in detecting these plaques were determined.Results: Thirty patients with MS with the mean age of 32.76±8.79 years and 30 healthy individuals with the mean age of 32.75±9.23 years were enrolled. Plaque within the brain was significantly higher by the conventional method (P<0.05). Ovoid lesions were greater in number in the conventional method group. More lesions were detected by the conventional method in the areas of periventricle, centrum semiovale and corpus callosum. Regarding the size of plaque, the minimum measurement was significantly lower in the conventional method group. The sensitivity of both methods was 100%. The specificity of conventional and DW MRI was 86.6% and 96.6%, respectively.Conclusion: In our study the sensitivity of both methods was the same but the specificity of DW MRI was higher. DW MRI may detect lesions which are not detectable by the routine methods.

Abstract: 

Introduction The staging of endometrial carcinoma is now performed based on postoperative pathological assessments. If the staging of the cancer could be possible with a preoperative non-invasive method, based on the International Federation of Gynecology and Obstetrics system, a more appropriate treatment protocol can be recommended for patients. This study was performed aimed to evaluate the diagnostic value of dynamic magnetic resonance imaging with Diffusion-weighted imaging in the staging of endometrial carcinoma, based on the postoperative histopathological reference standard. Methods This prospective study was conducted in 2016-2021 on 35 patients with endometrial cancer referred to Ghaem Hospital in Mashhad. The staging of endometrial cancer was performed based on MRI findings and pathological samples. The diagnostic value of MRI was calculated to distinguish stages 1 from 2 and 1A from 1B. The lesion dimensions in the MRI and the pathological samples were compared using independent-sample t-test and the lesion dimensions between different stages using the ANOVA test by SPSS (version 17). P<0. 05 was considered statistically significant. Results According to the findings of MRI and pathology, the majority of patients (93. 3%) were in stage 1, and 2 (6. 7%) were in stage 2. The sensitivity, specificity, positive and negative predictive value in differentiating stage 1 from stage 2 was 100%, and the sensitivity, specificity, positive and negative predictive value of MRI in differentiating IA from IB was 100%. Conclusion It seems that dMRI by DWI method can be used in the staging of endometrial tumors in the initial stages (stage 1 from 2 and IA from IB) with diagnostic value.

Abstract: 

Introduction: Wheat production is an important component for national food security. Studying the impact of future climate change on crop production is crucial to manage crop productivity. Improving our understanding of potential responses of the wheat plant to these changes while taking into consideration agricultural management is required for identifying best adaptation strategies. In Khorasan Razavi province, over 12% of the total cultivated wheat area is belong to Torbat-e Jam region. The current study aimed to quantify the potential impacts of climate change on irrigated wheat yield in the next 30 years for this region. Materials and Methods: In this study, information on regional cropping systems and meteorological data to study the effects of climate change were collected and finally, the impact of climate change on irrigated wheat yield was studied using simulation approaches under different climatic scenarios. Long-term daily weather data for future was generated using MarkSim weather generator software and the past meteorological data was gathered from regional weather station. Daily weather data generated by four scenarios RCP2. 6, RCP4. 5, RCP6, and RCP8. 5 for a 30-year period. The generated and measured daily weather data was used for running CERES-Wheat model. Finally, crop model outputs were used to interpret climate change impacts on wheat growth and production. Results and Discussion: The average air temperature during the wheat growth period showed that temperature increased about 1. 6% in the last five years compared to the first five years of the baseline 30-years period. Furthermore, the average annual precipitation for the last 5 years was 51 mm less than the first 5 years of the baseline period. The air temperature will increase about 1. 1, 1. 6, 1. 2 and 1. 8 ° C during the wheat growth period for the near future (30 years) under the RCP2. 6, RCP4. 5, RCP6, and RCP8. 5 scenarios, respectively. The precipitation over the wheat growth period will reduce by 9 and 8% under RCP4. 5 and RCP8. 5 scenarios, respectively. Furthermore, the precipitation over the growth period would increase by 11 and 17% under RCP2. 6 and RCP6 scenarios, respectively. The simulated wheat yield by the CEREC-Wheat model showed that the grain yield increased by 5% in the first 5 years compared to the last 5 years of the baseline period (1998-2017). The biomass production and harvest index (HI) also increased by 2. 3 and 1%, respectively. The increased dry matter production and HI can be due to temperature increase during the cold seasons, and more amount of precipitation during the reproductive growth. The simulated wheat growth during the near future period (2018-2047) showed that wheat growth cycle will reduce from 5 to 7 days under different emission scenarios. In the different soil textures, wheat yield under RCP2. 6, RCP4. 5, RCP6, and RCP8. 5 scenarios will increase by 24. 4, 19. 9, 24. 9 and 25. 2%, respectively. Furthermore, the highest and the lowest yield were obtained from clay and sandy clay loam soils, respectively. Under the different emission scenarios, the mean incident light and light use efficiency during the growth period would be increased by 20. 8-24. 6% and 5. 5-8. 3%, respectively, in projected climate change as compare to baseline period. Reducing irrigation water as amount of 300 m3 ha-1 and simultaneous increase in irrigation water use efficiency are possible incident scenarios for the near future. Based on these scenarios, simulations showed that grain yield increased 26. 3, 19. 6, 27. 2 and 25. 6 % under RCP2. 6, RCP4. 5, RCP6, and RCP8. 5 emission scenarios, respectively. Conclusions: The increased temperature and atmospheric CO2 concentration during the next 30 years will decrease wheat growth cycle, but increase the potential biomass and grain wheat yield under the different emission scenarios. The results showed that the increased yield production will occur under climate change conditions, even if the available irrigation water would decrease by 300 m3 ha-1.

Abstract: 

Purpose: The flourishing of political parties and currents is one of the signs of the degree of development in societies. In contemporary Iran and since the formation of the constitutional movement, political currents have gone through many ups and downs and it can be said that political currents in Iran after the Islamic Revolution have not yet reached the stage of institutionalization and stability. A characteristic feature of political currents in Iran is the divergence and division among political currents in recent decades, and this can be one of the reasons for the instability and cross-sectional and seasonal activity of political parties in Iran. Therefore, the necessity of leading research seeks to answer the question of how factors and divergence among political currents in Iran after the victory of the Islamic Revolution can be analyzed? And what are the scenarios for the advancement of political currents in Iran?Method: To answer this question, the method of causal-layer analysis, which is one of the qualitative methods in futures research, has been used.Findings: The research findings indicate that this divergence is due to a wide range of reasons from the level of causal systems (from the institutionalization of power to the formation of parties as elitist initiatives), worldview and discourse (from charismatic political authority to culture). Subsidiary-follower politics to myth-metaphor (Iranian individualism to belief in a strong state-weak society) can be analyzed.Conclusions: Three scenarios for the future of Iranian political currents can be considered: integration of currents as the security valve of the political system, the collapse of political currents in the traditional form, integration and consolidation in new social movements (virtualized parties).

Abstract: 

Introduction: The current research tries to identify the factors affecting the growth of professional sports clubs, which affect the development of the sports industry, by focusing on the strategic foresight of Iran's professional sports with a world-class approach.Methods: The data of this qualitative research, which had an exploratory-fundamental nature, was collected through targeted sampling with the snowball technique and based on semi-structured in-depth interviews with 20 experts in the field of professional sports clubs. A detailed interpretative understanding and preliminary and theoretical codings determined that professional sports clubs can build their desired future based on the requirements of shaping the future, by overcoming the weight of the past, the pressure of the present and in line with the tension of the future and the choice of proactive behavior in facing the future, set a perspective for it.Results: In this research, a total of fifteen main factors affecting the strategic foresight of Iran's professional sports with a world-class approach include: sports structure, foundational factors, hardware infrastructure, background factors, key stakeholders, development with a global approach, actors Key, human resource development, and possible consequences, global development consequences, preferred future, uncertainties, drivers and surprises were identified.Conclusion: In general, economic considerations are described in world-class professional sports more than anything else, although cultural elements also play a role in this field. Despite the importance of culture in globalization discourses in sports, economic considerations play an important role.
